The Medical Statement should be completed by your child’s doctor and returned to preschool by your child’s first day of school. Please attach a copy of your child’s immunization records with dates of all immunizations. Medical Statements are good for one year from the date of exam listed on the Medical Statement and are renewed at expiration.
